CenterWell August Sunnyside Calendar

CenterWell is a primary care clinic for seniors with Medicare Advantage health plans that provides longer appointment times and shorter wait times. Our board-certified physicians see an average of 12 patients per day, allowing us to spend more time and provide the quality each patient deserves. Our care team consists of board-certified physicians, Integrated pharmacists, Registered Nurse Care Coaches, Behavioral Health specialists, and Resource Coordinator to assist with any social determinants of health barriers. The entire team is on-site to assist our patients with medical, emotional, and social issues and it’s all for free when they become part of the CenterWell primary care family. We have been in the market for about two years now and service 12 locations in the surrounding Houston area.
